In 1988, Mahoney and his then-spouse, Mary Jo West, adopted a baby from Santa Rosa de Cop·n, Honduras, an adoption arranged personally by Mother Teresa of Calcutta who later visited her namesake in Phoenix in 1989. Maria Teresa (or Molly) is now sixteen years old and lives in Phoenix.

In 1998, Mahoney moved to Costa Rica where he launched an experiment in "social entrepreneurship" - establishing a small business whose profits fund human relief or social justice. The Nuestra Familia Escuela de Idiomas (established in 1999) offered courses in English and Spanish and used its profits to fund two orphanages in Heredia (outside the capital city of San JosČ) and the staff at the language school to teach the children. The program was later adopted by three multinational companies led by Motorola. Today all of Costa Rica's public orphanages are equipped with computers and interactive software.
